Factors influencing poor hygiene among secondary School students in Nigeria

CHAPTER ONE/INTRODUCTION

According to Ministry of Public Health and Sanitation and the Ministry of Education (2009), hygiene is the practice of keeping oneself and ones surroundings clean so as to prevent illness or the spread of preventable diseases. The practice may include hand washing with soap, proper use of sanitation facilities, proper disposal human waste and menstrual hygiene management.

Since its inception in 2008, The Global Hand Washing Day has been reinforcing the call for improved hygiene practices worldwide (UNICEF and WHO, 2008). Its key vision is the implementation of a local and global culture of hand washing with soap as a major strategy in reducing diarrhea infections by 30-50% (Cairncross and Valdmanis, 2006). However, studies have shown that globally, the rate at which hands are washed with soap ranges from 0-34% (Parker, 1993).This is a very insignificant number compared to the total world population.

According to Crofts and Fisher (2011), menstrual hygiene management has been an issue for almost half of the world’s adolescent girls. It has been a cause of shame, stigma, and school absenteeism and contributes greatly to reproductive tract infections. Addressing these hygiene practices particularly in public schools can bring the much needed international attention to focus on these neglected issues. Schools, particularly those in rural areas often have inadequate water, toilets, hand washing soap and hand washing facilities making it difficult for some students to practice proper hygiene. Boys and girls are likely to be affected in different ways by this inadequacy and this may contribute to unequal learning opportunities. Sometimes girls fail to practice proper hygiene as was observed by Parker (1993) in a study carried out in  Libode district in Zimbabwe where some girls reported dropping used sanitary towels in the pit latrines or throwing them in the nearby bushes since their sanitary towel disposal bins were always full and dirty. A study carried by Parker (1993) in West Bengal showed that girls missed school during menstruation since they could not use the sanitary towel disposal bins which were dirty and smelly. These practices interfere with the achievement of one of the Millennium Development Goals on ensuring environmental sustainability since this disposal of used sanitary towels is not ecologically friendly. Another Millennium Development Goal on the promotion of gender equality will also not be achieved since girls, unlike boys, miss school during menstruation period.

In most public schools, latrine to student ratio is a core concern with hundreds of students sharing a single toilet thus affording inadequate privacy especially for the girls. This is in contrast to recommendation by The Ministry of Public Health and Sanitation and Ministry of Education (2009) which recommend a ratio of 1 toilet for every 25 girls and 1 toilet for every 30 boys in order for the sanitation facilities to provide adequate privacy to all students. A study conducted in Machakos showed that an average of 64 students was sharing one toilet (WHO, 2009). Another study carried out by Curtis and Cairncross (2003) showed that in most public schools, toilets were divided into cubicles with no doors and had an open roof. Most of the toilets were dirty with feces on the walls and urine on the floor. Such conditions do not favor proper hygiene practices. There is therefore need to build separate gender-appropriate toilets that provide privacy, adequate hand washing water, soap and disposal facilities for the entire school community. In addition, proper hygiene practices should be instigated in public schools since sanitary conditions and basic personal hygiene practices such as hand washing using soap, proper

disposal of used sanitary towels and proper use of toilets are still not widely practiced among students. Schools should provide an enabling environment where the students can learn these practices and implement them both in school and even at home.

 Objectives of the Study

The study was guided by the following objectives

  1. To establish whether availability of safe water influences implementation of hygiene practices in public secondary schools in Ebonyi state
  2. To establish whether availability of soap influences implementation of hygiene practices in public secondary schools in Ebonyi state
  3. To establish the relationship between availability of toilets and the implementation of hygiene practices in public secondary schools in Ebonyi state
  4. To investigate the relationship between availability of sanitary towel disposal bins and implementation of hygiene practices in public secondary schools in Ebonyi state

Research Questions

The study addressed the following research questions

  1. To what extent does the availability of water influence implementation of hygiene practices in public secondary schools in Ebonyi state?
  2. To what extent does the availability soap influence implementation of hygiene practices in public secondary schools in Ebonyi state?
  3. To what extent does the availability of toilets influence implementation of hygiene practices in public secondary schools in Ebonyi state?
  4. To what extent does the availability of sanitary towel disposal bins influence implementation of hygiene practices in public secondary schools in Ebonyi state?

Hypotheses

  1. There is no significant relationship between availability of water and implementation of hygiene practices in secondary schools in Ebonyi state
  2. There is no significant relationship between availability of soap and implementation of hygiene practices in secondary schools in Ebonyi state
  3. There is no significant relationship between availability of toilets and implementation of hygiene practices in secondary schools in Ebonyi state
  4. There is no significant relationship between availability of sanitary disposal bins and implementation of hygiene practices in secondary schools in Ebonyi state

   Scope of the Study

The research study confined itself to the factors influencing implementation of hygiene practices in public secondary schools in Ebonyi state namely availability of soap, safe water, toilets and sanitary towel disposal bins. The study may not have been generalized to cover tertiary institutions and universities since factors influencing implementation of hygiene practices in these institutions may be significantly different.

Data was collected from form two and three students only but not from form one and four students. This is because the form two and three students had stayed in the school long enough and had familiarized themselves with the school environment and therefore knew where most of sanitation facilities were unlike the form one students who were still be in the process of being oriented in the school. The form four students were revising for their mock examination and therefore seemed too busy to fill the questionnaires.

There are moderating variables that may influence implementation of hygiene practices. These include students’ attitudes, background, beliefs, knowledge and perception towards hygiene but due to limited time and other resources, this study did not collect data on them.

  Significance

The findings of this study may be of help to several groups of people: Firstly, it may provide the curriculum planners and developers and all the stakeholders in the education sector with a better understanding of factors influencing implementation of hygiene practices in schools. The curriculum planners may then use the study findings to develop appropriate national standards of toilets and other hygiene facilities that are relevant to secondary schools in Kenya.

Secondly, the ministry of education may use the study findings to develop a school program with guidelines for hygiene practices. These guidelines may be approved by the ministry and send to teacher training colleges where they may be used to prepare teachers and help them develop

 

curricular materials related to hygiene. The trained teachers may then use the materials and the guidelines to incorporate the theme into their subjects and encourage students to practice the knowledge gained both in school and at home.

Thirdly, the community health workers may use the research findings to educate the general public on good hygiene practices and factors that influence their implementation. They may as well use the findings to persuade individuals, families and social groups to adopt new, healthier and better hygiene practices.

Fourthly, the research findings may also provide a foundation for policy makers and project managers to make rational decisions on improving adolescent reproductive health both in school and in the communities.

Finally, heads of learning institutions and the teaching and non-teaching staff may use the research findings to re-assess hygiene facilities in their institutions and perhaps apply recommendations made by the researcher to better these facilities and also emphasize on improving hygiene practices in their own schools.

CHAPTER 1/INTRODUCTION

Nutritional supplements are preparations intended to supplement the diet and provide nutrients (Aina and Ojedokun, 2014). They include vitamins, minerals, fiber, fatty acids, or amino acids, which may be missing or may not be consumed in sufficient quantities in a person’s diet (Aina and Ojedokun, 2014). Caballero (2006) also defines dietary supplements as: “products (other than tobacco) intended to supplement the diet that bears or contains one or more of the following dietary ingredients: vitamins, minerals, amino acids, herbs or other botanical”. The definition by the National Institute of Health’s Office of Dietary Supplements gives a definition which also agrees with the two previously given: it defines dietary supplements as “products intended to supplement the diet and which contain one or more dietary ingredients (including vitamins, minerals, herbs or other botanicals, amino acids and other substances) or their constituents” (Gardiner et al., 2007).

Herbal supplements are one of the notable types of dietary supplements, from the definitions given above (Caballero, 2006; Gardiner et al., 2007; Aina and Ojedokun, 2014). In 2005, the World Health Organization (WHO), defined herbal supplements as plant-derived materials or preparations with therapeutic health benefits, which contain either raw or processed ingredients from one or more plants (WHO, 2005). Also, according to Staines (2011), herbal supplements may also be called herbal medicines, remedies, supplements, botanicals, or phytomedicines.

Individuals of practically all age groups make use of these supplements for a variety of reasons such as, for weight loss, fertility, combating colds, to delay ageing, enhancing brain function, body building, etc (National Health Service, UK, 2011). A recent research at the University of Lagos, Nigeria, involving students of the College of Medicine (Aina and Ojedokun, 2014), found that about 86% of the subjects surveyed had made use of dietary supplements. Also, a community-based survey in a rural population in the United States, involving 526 subjects (211 male and 314 female, with ages ranging from 18 to above 65 years, and including health workers, students, retired adults, unemployed, etc.), found that 71.5% of the subjects preferred dietary supplements or herbal products to prescription medication and over-the-counter (OTC) medicines for treating medical conditions and/or to maintain health (Owens et al., 2014). With these in mind, questions easily arise regarding how much knowledge people actually have about these supplements, in terms of how they should be used, appropriate quantities that should be taken, awareness of their safety, etc.  However, according to Aina and Ojedokun (2014), “there is limited number of research on dietary supplement use in developing countries like Nigeria.” This may imply poor awareness and/or an insignificant use of dietary supplement in our country, thus necessitating the question as to awareness and use of dietary supplements among the Nigerian populace.

Recent years have seen a massive boom in supplement use, as products that were once the preserve of specialist health food stores have become available alongside provisions and foodstuffs in the supermarket and on the internet (National Health Service, UK, 2011). As availability has grown, so have sales. In 2009, the UK market for dietary supplements and vitamins was worth more than £670 million (National Health Service, UK, 2011).

This study is focused on undergraduates – youths, and perhaps it would be useful to take a brief look at the youths of Nigeria. The National Youth Development Policy (2001) defined youth as people aged 18-35 years. It is a stage in life when a person is young, especially the time before a person becomes an adult (Hornby, 2001). Omede (2012) opined that: “psychologically, it refers to the stretch of time in-between childhood and adulthood, and characteristically, youths at this stage of growth and development are biologically known to be full of libidinal energy and ideas, and are also numerically known to outnumber the middle-aged and the aged (Omede, 2012). The Nigerian youths, according to Anasi (2010), constituted about 40% of the more than 140 million people in the country (Anasi, 2010).

A vast population of these youths are concentrated in institutions of higher learning, thus making these institutions a vital venue for researches concerning youths. Higher institutions are obviously the last center of massive convergence of youths before they are released into the outside world. Hence, it would be useful and fundamental to carry out such a research as this concerning dietary supplements (which are increasingly becoming a matter of concern worldwide), among undergraduates, so that corrective actions can be taken against any erroneous or deleterious attitudes, misinformation or misuse of these supplements, which they may have acquired or be involved in.

STATEMENT OF PROBLEM

Dietary supplements are used by so many, in one form or the other. Some even use or have used several different supplements at a time (Aina and Ojedokun, 2012). Researches indicate a rise in their usage worldwide (Strachan, 2009; Elder and Nisly, 2011). Unfortunately, a lot of these users are largely ignorant of appropriate usage of the supplements. For instance, in the research conducted by Aina and Ojedokun, (2014), “almost two-thirds of the students said dietary supplements can be used to substitute natural nutrients derived from foods, which indicates some knowledge deficiency, because dietary supplements are supposed to supplement and not substitute natural nutrients from food” (Aina and Ojedokun, 2014). Also, several of these supplements contain substances which may elicit dangerous side effects or have adverse reactions with drugs or other foods which may be ingested with them (Owens et al., 2014). Hence, it is increasingly becoming important to find out how much users of these dietary supplements know about them (including their side effects or any dangers associated with their usage), how often they make use of them, and the diverse attitudes individuals have towards these supplements.

OBJECTIVES

General objective

The general objective of this research is to assess the knowledge, perception and use of dietary supplements among undergraduates.

Specific objectives:

The specific objectives are to:

  1. assess the level of knowledge of undergraduates about dietary supplements.
  2. ascertain their perception towards the use of dietary supplements.
  3. determine the use of dietary supplements.
  4. assess the relationship between dietary supplement use, knowledge and perception.

SIGNIFICANCE OF THE STUDY

This study will add to the body of knowledge on dietary supplements in Nigeria, thereby increasing awareness about these supplements, including their appropriate use, harmful ingredients contained in some, side effects, etc. Results from this research could help nutrition and health professionals to better understand the situation of Nigeria, in terms of dietary supplements, particularly in the aspects of the knowledge of the youths about dietary supplements, including appropriate use, side effects, and their various attitudes towards these supplements.

Besides these, results from this study could stimulate individuals and corporate bodies towards creating more awareness and providing reliable knowledge to the youths and the entire nation vis-à-vis dietary supplements, so as to attempt to positively influence them in order to encourage appropriate use, thereby preventing detrimental side-effectsDownload Full Material-N5000

Abstract

Background:

Adolescence is a period of overwhelming changes and challenges, which expose the adolescents to high-risk behaviors. Risky sexual relationship is one of these behaviors that entails physical risks and psychosocial harms. Various factors have been recognized to shape sexual behaviors in adolescents. This paper is an attempt to investigate the factors contributing to high-risk sexual behaviors in Iranian adolescent girls.

Methods:

A literature review of the research published by Iranian authors, in Farsi or English language in local and foreign journals, was conducted using PubMed, Science Direct, Scopus, Scientific Information Database (SID), IranMedex, IranDoc, and Google Scholar. The search in each database included all the years covered at that time using keywords such as “sexual, adolescents, and Iran”, and continued using other keywords such as “sexual behavior, high-risk behavior, sexual risk and reproductive behavior” individually and in combination

Results:

Sixteen published articles were identified. Factors contributing to high-risk sexual behaviors in girls can be divided into four general groups including personal, family, peer, school and community.

Conclusion:

Regarding the identified risk and protective factors, appropriate individual, family and school-based interventions can be designed and implemented to strengthen protective factors. While individual and family factors are considered more in research, factors related to peers, school and community have received less attention. Since social values, beliefs and norms are important factors in formation of sexual behaviors, further research regarding these factors is suggested.

Scientists and public health officials are working hard to identify the source of the SARS-CoV-2 which is the virus that causes COVID-19. The initial transmission appeared to be from an animal source, but there has been person-to-person transmission in countries.
The virus can spread from one person to another, most likely through droplets of saliva or mucus

The first case in Nigeria was confirmed on Friday, February 27, 2020 in Lagos in a business Italian traveller who arrived Nigeria on February 25, 2020 and became symptomatic on February 26, 2020. Contact tracing was initiated by the goverment to identify everyone who has come in contact with the confirmed case.The Government of Nigeria, through the Federal Ministry of Health, has been strengthening measures to ensure Nigeria is prepared in the event of an outbreak.

To reduce the risk of infection or spread of COVID-19, members of the public were advised to adhere to the following hand and respiratory hygiene measures:

• Wash your hands regularly with soap under running water and use hand sanitisers frequently
• Cover your mouth and nose properly with tissue paper when sneezing and/or coughing and immediately dispose of the tissue in a covered waste bin. After this, wash your hands with soap and water or use hand sanitiser

• You may also cough into your elbow if disposable tissue is not available. You are strongly advised not to re-use handkerchiefs

• If you become sick while travelling, you should avoid close contact with other people while you are symptomatic and cover your mouth and nose with a face mask

• Avoid close contact with anyone showing symptoms of respiratory illness such as coughing and sneezing

• Healthcare workers are always advised to observe standard infection prevention and control measures when attending to patients and take a travel history
